Aerodrome Safety Incident Investigation
Despite the aviation industry's best intentions and significant efforts, safety incidents may occur. When they do, a modern safety management approach demands that we learn from the occurrence to prevent a repeat event in the future. Aviation safety regulations require airport operators to look at the bigger picture and identify latent as well as active factors.
Extracting information from the incident environment and analysing it requires a wide range of skills and knowledge to arrive at a coherent set of conclusions and a workable list of recommendations.
During this 3-day course you will gain the knowledge and skills required to conduct safety incident investigations in the aerodrome environment, and therefore support the ongoing operation and development of your aerodrome’s safety management system. The course will build a foundation in incident investigation by covering the regulatory environment, essential safety management principles, and incident investigation management. Upon this, the course will delve into data and information gathering techniques, preliminary and in-depth analysis tools, developing recommendations and report writing considerations.
Attendees will be encouraged to participate in and contribute to group discussions and team exercises using their personal experiences and knowledge.

Trainer
Daniele Occhiato
Daniele holds a Diploma degree in aerospace engineering and a Master Degree in Aviation Safety. He is an aviation consultant and project engineer for airsight with extensive expertise in airport operations, aircraft operations, apron management services, ground handling, safety management systems and emergency response planning as well as auditing and compliance monitoring at aerodromes.
For more than 10 years, Daniele has been involved in the development and management of Safety Management Systems and Compliance monitoring systems in both complex (international hub) and less complex aerodrome realities.
At airsight, he is involved in the conduction of safety assessments at airports, focusing on airport and aircraft operations related issues as well as safety management systems.
Daniele is very knowledgeable in the international aerodrome regulations (ICAO/EASA), their national implementation, as well as industry’s best-practices and works for airsight as a lead auditor. He has been part of the auditor team for some ACI APEX (Airport Excellence Programmes) reviews at international airports.
Caroline Schlemmer, M.Sc.
Caroline Schlemmer is a consultant in airsight’s “Airport Safety & Operations” department. She works on safety assessments, SMS implementation and projects related to the development of airport Concepts of Operations (CONOPS). Further, Caroline is an instructor for the Aircraft Accident and Incident Investigation training course.
She completed her master’s degree at the Technical University of Berlin (TU Berlin) in 2016, after conducting her thesis at the University of Washington, Seattle. Afterwards she started working as a lecturer and research assistant at the Chair of Flight Mechanics, Flight Control and Aeroelasticity of TU Berlin. Her master’s thesis on modelling and simulating longitudinal control surfaces of a flexible aircraft formed the way for her further research on aeroservoelasticity. She presented her research results on national and international conferences and in the corresponding papers. As a university lecturer for flight mechanics, she gained comprehensive experience in teaching and sharing her knowledge in German and English.
For airsight, she has participated in safety assessments related to non-compliant airfield requirements like non-compliant slopes. She also supported case studies assessing the feasibility of constructions sites in the vicinity of active runways. More recently, Caroline is involved in a jetblast study aiming to ensure accurate data through on-site measurements and the definition of mitigation measures in the post-processing phase.

Dan Parsons
Dan is an airport operations and safety professional whose experience covers both industry and regulatory sides of the industry. Following broad aviation studies, Dan began specialising in aerodrome operation in 2005 and has since expanded his experience to cover broad airport operations including technical inspections and surveys, facilities management, aviation security, contract management, commercial management and infrastructure development.
Training and development have always been a big part of Dan's work starting with aerodrome reporting officer training, team development as well as mentoring. Dan is passionate about practical safety management and safety assurance in particular. He has, unfortunately, had to investigate a number of significant airport-related events including personnel injuries, aircraft write-offs and security threats.
